Indication: Used for the treatment of Gastroesophageal Reflux Disease (GERD)
Mechanism of Action: Dexlansoprazole is a proton pump inhibitor that reduces gastric acid secretion by specifically inhibiting the (H+, K+)-ATPase enzyme at the secretory surface of gastric parietal cells. This blocks the final step in acid production.
Usage and Safety:
Dosage: As directed by a healthcare provider.
Common Side Effects: Diarrhea, abdominal pain, nausea, upper respiratory tract infection, vomiting, flatulence.
Less Common Side Effects: Anemia, lymphadenopathy, angina, arrhythmia, bradycardia, chest pain, edema, myocardial infarction, palpitations, tachycardia, ear pain, tinnitus, vertigo, goiter, eye irritation, eye swelling, abdominal discomfort, abdominal tenderness, abnormal feces, anal discomfort, Barrett’s esophagus, bezoar, abnormal bowel sounds, breath odor, colitis microscopic, colonic polyp, constipation, dry mouth, duodenitis, dyspepsia, dysphagia, enteritis, eructation, esophagitis, gastric polyp, gastritis, gastroenteritis, gastrointestinal disorders.
Drug Interactions: May interact with rilpivirine, atazanavir, nelfinavir, saquinavir, warfarin, methotrexate, digoxin, iron salts, erlotinib, dasatinib, nilotinib, mycophenolate mofetil, ketoconazole/itraconazole, tacrolimus, tetrahydrocannabinol (THC), CYP2C19 or CYP3A4 inducers (e.g., St. John’s Wort, rifampin), and CYP2C19 or CYP3A4 inhibitors (e.g., voriconazole).
Indications for Use
For Patients Aged 12 and Older:
To heal all grades of erosive esophagitis (EE) for up to 8 weeks.
To maintain healing of EE and relieve heartburn for up to 6 months.
For the treatment of heartburn associated with symptomatic non-erosive GERD for up to 4 weeks.
Contraindications
Do Not Use:
In patients with known hypersensitivity to dexlansoprazole or any excipient in the product.
With rilpivirine-containing products.
In patients with severe hepatic impairment.
Precautions:
General Precautions: Proton pump inhibitors (PPIs) may increase the risk of Clostridium difficile-associated diarrhea, especially in hospitalized patients. Use the lowest effective dose and shortest duration of therapy.
Warnings
Warning 1: Prolonged use of PPIs may increase the risk of osteoporosis-related bone fractures (hip, spine, wrist). Use the lowest effective dose and shortest duration, and manage patients at risk for osteoporosis-related fractures according to guidelines.
Warning 2: Consider checking serum magnesium levels before starting long-term therapy, especially if taking digoxin, diuretics, or other medications that can cause hypomagnesemia.
Warning 3: Long-term treatment may lead to vitamin B12 malabsorption and deficiency.
