How It Works:
Omeprazole, a proton pump inhibitor, reduces gastric acid secretion by irreversibly blocking the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, which is the proton pump of the gastric parietal cells. This action inhibits both basal and stimulated acid secretion, regardless of the stimulus. Sodium bicarbonate helps neutralize stomach acid and enhances the effectiveness of omeprazole.

Indication:
Omeprazole is indicated in adults for:
Treatment of erosive reflux esophagitis
Long-term management to prevent relapse in patients with healed esophagitis
Symptomatic treatment of gastroesophageal reflux disease (GERD)
When Not to Use:
Omeprazole is contraindicated in:
Patients with known hypersensitivity to any component of the formulation or substituted benzimidazoles
Patients taking nelfinavir

Precaution:
A symptomatic response to therapy with omeprazole does not rule out the possibility of gastric malignancy.

Warning 1:
Long-term use of omeprazole may be associated with atrophic gastritis, as noted in gastric corpus biopsies.
Warning 2:
Sodium bicarbonate should be used with caution in patients with Bartter’s syndrome, hypokalemia, respiratory alkalosis, and issues with acid-base balance. Long-term use with calcium or milk may cause milk-alkali syndrome.
Warning 3:
Omeprazole powder for oral suspension contains sodium in the form of sodium bicarbonate, which should be considered for patients on a sodium-restricted diet.
