How It Works:
Vonoprazan is a potassium-competitive acid blocker (P-CAB) that inhibits the H+, K+-ATPase enzyme in a reversible and potassium-competitive manner. Unlike traditional proton pump inhibitors, it does not require activation by acid. It binds to the acid production site on gastric parietal cells, providing long-lasting inhibition of gastric acid production. This mechanism helps prevent mucosal damage in the upper gastrointestinal tract.

Side Effects:
Common: Diarrhea, constipation, nausea, abdominal distension
Less Common: Drug hypersensitivity (including anaphylactic shock), drug eruption, urticaria, hepatotoxicity, jaundice, rash
Laboratory Abnormalities: Increased gamma-glutamyl transferase, AST, ALT, ALP, LDH, and eosinophilia
Other: Edema
Drug Interactions:
CYP3A4 inhibitors: Clarithromycin, Itraconazole
Digoxin, Methyldigoxin
Tyrosine kinase inhibitors: Gefitinib, Nilotinib, Erlotinib
Indication:
Vonoprazan is indicated for:
Gastric ulcer
Duodenal ulcer
Reflux esophagitis
Prevention of recurrence of gastric or duodenal ulcers during low-dose aspirin or NSAID administration
When Not to Use:
Vonoprazan is contraindicated in:
Patients with hypersensitivity to Vonoprazan or any excipient of the product
Patients receiving atazanavir sulfate, nelfinavir, or rilpivirine hydrochloride

Precaution:
Monitor the course of the disease closely during treatment. Use the minimum effective dose necessary based on the condition of the disease.
Scivono Tablets Warnings:
Warning 1:
For maintaining healing of reflux esophagitis, Vonoprazan should only be used in patients with recurrent or persistent symptoms. Avoid use in patients who do not need maintenance therapy.
Warning 2:
If healing is maintained over a long period and there is no risk of recurrence, consider reducing the dose from 20 mg to 10 mg or discontinuing the medication.
Warning 3:
Use Vonoprazan with caution in patients with renal disorders, as delayed excretion may increase blood concentration of the drug.
