How It Works:
Omeprazole reduces gastric acid secretion by irreversibly blocking the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase), also known as the proton pump, in the gastric parietal cells. It belongs to a class of anti-secretory compounds known as substituted benzimidazoles, which do not exhibit anticholinergic or H2 histamine antagonist properties. This mechanism inhibits both basal and stimulated acid secretion regardless of the stimulus. Sodium bicarbonate acts as a buffering agent to neutralize excess stomach acid.

Warning 1:
Atrophic gastritis has been observed in gastric corpus biopsies from patients undergoing long-term omeprazole therapy.
Warning 2:
Sodium bicarbonate should be used with caution in patients with Bartter’s syndrome, hypokalemia, respiratory alkalosis, or acid-base balance issues. Long-term use with calcium or milk can lead to milk-alkali syndrome.
Warning 3:
Omeprazole powder for oral suspension contains sodium in the form of sodium bicarbonate, which should be considered for patients on a sodium-restricted diet.
