How it Works: Omeprazole inhibits gastric acid secretion by irreversibly blocking the H+/K+ ATPase enzyme in gastric parietal cells, reducing both basal and stimulated acid production.
Usage and Safety:
Dosage: As prescribed by your healthcare provider.
Side Effects:
Common: Headache, diarrhea, constipation, abdominal pain, nausea/vomiting, flatulence.
Uncommon: Dizziness, paresthesia, somnolence, insomnia, vertigo, increased liver enzymes, rash, pruritus, urticaria, malaise.
Rare: Reversible mental confusion, agitation, aggression, depression, hallucinations, gynecomastia.
Drug Interactions: May interact with ketoconazole, itraconazole, diazepam, citalopram, imipramine, clomipramine, phenytoin, atazanavir, digoxin, clopidogrel, posaconazole, erlotinib, saquinavir, tacrolimus, methotrexate, clarithromycin, and CYP2C19 or CYP3A4 inhibitors/inducers (e.g., rifampicin, St. John’s wort).
Indications:
For Use: Treatment of erosive reflux esophagitis, long-term management of healed esophagitis to prevent relapse, and symptomatic treatment of GERD.
Contraindications
Do Not Use: In patients with hypersensitivity to any component of the formulation or to substituted benzimidazoles. Not to be used with nelfinavir.
Precautions:
Precaution: Long-term use may reduce vitamin B12 absorption. Monitor patients with risk factors for vitamin B12 deficiency.
Warnings:
Warning 1: Rule out malignancy in cases of suspected gastric ulcers, as treatment may mask symptoms and delay diagnosis.
Warning 2: Long-term use, especially in high doses, may increase the risk of fractures in the hip, wrist, or spine, particularly in the elderly or those with other risk factors.
Warning 3: Dose adjustments may be necessary for patients with impaired hepatic function due to increased bioavailability and half-life of omeprazole.
