How It Works:
Omeprazole inhibits gastric acid secretion by irreversibly blocking the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, which is the proton pump in gastric parietal cells. This results in a dose-dependent reduction of both basal and stimulated acid secretion.

Indication:
Omeprazole is indicated for adults for the treatment of:
Erosive reflux esophagitis
Long-term management of patients with healed esophagitis to prevent relapse
Symptomatic treatment of gastroesophageal reflux disease (GERD)
When Not to Use:
Omeprazole is contraindicated in:
Patients with known hypersensitivity to omeprazole or substituted benzimidazoles
Patients taking nelfinavir

Precaution:
Omeprazole may reduce vitamin B12 absorption due to hypo- or achlorhydria. This should be considered in patients with low body stores or risk factors for vitamin B12 deficiency on long-term therapy.

Warning 1:
When a gastric ulcer is suspected, malignancy should be ruled out as treatment may alleviate symptoms and delay diagnosis.
Warning 2:
Long-term use of proton pump inhibitors, especially in high doses and for durations exceeding one year, may slightly increase the risk of fractures (hip, wrist, spine), particularly in the elderly or those with other risk factors.
Warning 3:
Consider reducing the dose of omeprazole in patients with impaired hepatic function due to increased bioavailability and prolonged half-life.
