How It Works:
Ondansetron is a selective antagonist of the serotonin 5-HT3 receptor. While its exact mechanism in preventing chemotherapy-induced nausea and vomiting is not fully understood, it is thought to work by blocking the 5-HT3 receptors involved in the vomiting reflex triggered by serotonin release from enterochromaffin cells in the small intestine. Ondansetron may also affect the central nervous system by blocking 5-HT3 receptors in the chemoreceptor trigger zone. Its antiemetic effects in postoperative nausea and vomiting are similarly attributed to its selective receptor antagonism, though the precise mechanisms are not well defined.

Side Effects:
Common: Diarrhea, headache, fever, constipation, rash
Cardiovascular: Rare cases of angina, ECG changes, hypotension, tachycardia, QT prolongation, ST segment depression, bradycardia
Hepatic: Transient elevations in AST and ALT, exceeding twice the upper limit of normal in about 5% of patients, not related to dose or duration
Neurological: Rare cases of extrapyramidal reactions, grand mal seizures
Other: Rare cases of hypokalemia, headache, drowsiness/sedation, injection site reactions, fever, cold sensation, pruritus, paresthesia
Local Reactions: Pain, redness, burning at the injection site
Lower Respiratory: Hiccups
Skin: Urticaria, Stevens-Johnson syndrome, toxic epidermal necrolysis
Eye Disorders: Transient blindness
Neurological: Oculogyric crisis, dystonic reactions, transient dizziness

Indication:
Prevention of nausea and vomiting associated with initial and repeat courses of emetogenic cancer chemotherapy
When Not to Use:
Contraindicated in patients with known hypersensitivity to Ondansetron or any of its components
Concomitant use with apomorphine is contraindicated due to the risk of profound hypotension and loss of consciousness
Seroblock Tablets Precautions:
Precaution:
Hypersensitivity reactions, including anaphylaxis and bronchospasm, have been reported in patients with hypersensitivity to other 5-HT3 receptor antagonists.
Seroblock Tablets Warnings:
Warning 1:
Ondansetron can prolong the QT interval in a dose-dependent manner. Avoid in patients with congenital long QT syndrome. ECG monitoring is recommended for patients with electrolyte abnormalities (e.g., hypokalemia or hypomagnesemia), congestive heart failure, bradyarrhythmias, or those on other QT-prolonging medications.
Warning 2:
Ondansetron may mask progressive ileus and gastric distention following abdominal surgery or chemotherapy-induced nausea and vomiting.
Warning 3:
Ondansetron does not stimulate gastric or intestinal peristalsis and should not be used as a substitute for nasogastric suction.
