How It Works:
Omeprazole reduces gastric acid secretion by irreversibly inhibiting the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, also known as the proton pump, in the gastric parietal cells. This mechanism results in a dose-dependent suppression of both basal and stimulated acid secretion, regardless of the stimulus.

Contraindicated in patients with known hypersensitivity to any component of the formulation or substituted benzimidazoles
Must not be used with nelfinavir

Precaution:
Omeprazole, like other acid-blocking medications, may impair vitamin B12 (cyanocobalamin) absorption due to reduced gastric acid levels. This is important to consider in patients with low body stores or those at risk of impaired vitamin B12 absorption during long-term therapy.

Warning 1:
When a gastric ulcer is suspected, malignancy should be ruled out as omeprazole may alleviate symptoms and potentially delay diagnosis.
Warning 2:
Prolonged use or high doses of proton pump inhibitors (beyond one year) may modestly increase the risk of hip, wrist, and spine fractures, particularly in the elderly or those with other risk factors.
Warning 3:
In patients with impaired hepatic function, dose adjustments may be necessary due to increased bioavailability and half-life of omeprazole.
