How It Works:
Omeprazole inhibits gastric acid secretion by irreversibly blocking the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, known as the proton pump, in the gastric parietal cells. This action is dose-related and leads to inhibition of both basal and stimulated acid secretion, regardless of the stimulus.

Indication:
Treatment of erosive reflux esophagitis
Long-term management to prevent relapse in patients with healed esophagitis

When Not to Use:
Contraindicated in patients with known hypersensitivity to any component of the formulation or substituted benzimidazoles
Must not be used with nelfinavir

Precaution:
Omeprazole may reduce the absorption of vitamin B12 due to hypo- or achlorhydria. This should be considered in patients with reduced body stores or risk factors for vitamin B12 deficiency, especially with long-term use.

Warning 1:
When gastric ulcers are suspected, exclude the possibility of malignancy, as treatment may alleviate symptoms and delay diagnosis.
Warning 2:
Long-term use of proton pump inhibitors, especially in high doses and for over a year, may modestly increase the risk of hip, wrist, and spine fractures, particularly in the elderly or those with other risk factors.
Warning 3:
Consider reducing the dose in patients with impaired hepatic function due to increased bioavailability and half-life of omeprazole.
