Used For: Gastroesophageal Reflux Disease (GERD)
How it Works: Omeprazole irreversibly inhibits the hydrogen/potassium adenosine triphosphatase (H+/K+ ATPase) enzyme system, blocking gastric acid secretion from parietal cells. This results in reduced acid production, both basal and stimulated, regardless of the stimulus.
Losec Capsules Usage and Safety:
Dosage: Omeprazole
Side Effects:
Common: Headache, diarrhea, constipation, abdominal pain, nausea/vomiting, flatulence.
Uncommon: Dizziness, paraesthesia, somnolence, insomnia, vertigo, increased liver enzymes, rash, pruritis, urticaria, malaise.
Rare: Reversible mental confusion, agitation, aggression, depression, hallucinations (mainly in severely ill patients), gynecomastia.
Drug Interactions: Interacts with ketoconazole, itraconazole, diazepam, citalopram, imipramine, clomipramine, phenytoin, atazanavir, digoxin, clopidogrel, posaconazole, erlotinib, saquinavir, tacrolimus, methotrexate, clarithromycin, and CYP2C19 or CYP3A4 inhibitors/inducers (e.g., rifampicin, St. John’s wort).
Indication: For the treatment of GERD, including erosive reflux esophagitis, long-term management to prevent relapse, and symptomatic relief of GERD.
When Not to Use: Contraindicated in patients with hypersensitivity to omeprazole or substituted benzimidazoles. Should not be used with nelfinavir.
Losec Capsules Precautions:
Precaution: Long-term use may reduce vitamin B12 absorption due to hypo- or achlorhydria. Consider this in patients with reduced body stores or risk factors for vitamin B12 deficiency.
Losec Capsules Warnings:
Warning 1: Exclude malignancy when gastric ulcer is suspected, as treatment may alleviate symptoms and delay diagnosis.
Warning 2: Long-term, high-dose PPI use may increase the risk of fractures (hip, wrist, spine), particularly in the elderly or those with other risk factors.
Warning 3: Adjust dose in patients with impaired hepatic function, as bioavailability and half-life may be increased.
