Apixaban is a novel, low-molecular-weight drug that indirectly inhibits Factor Xa, an important enzyme involved in the coagulation pathway, by binding to the active site of the factor Xa receptor. Factor Xa’s role in catalyzing the formation of thrombin, which in turn converts fibrinogen to fibrin, the protein backbone of blood clots, makes it an important target for anticoagulants. Apixaban’s specificity for Factor Xa allows it to inhibit the formation of thrombin and therefore thin the blood and prevent excessive clot formation without interfering with other enzymes in the coagulation cascade. This makes it a safe and effective anticoagulant and therefore it is used to prevent blood clots from forming or growing in the blood vessels in patients who have atrial fibrillation, recent pulmonary embolism, or who have recently had a total hip or knee replacement.
